4. Information re: other malignancies
5. Imaging Reports
a)chest X-ray
b)CT scan, (brain, thorax, abdomen, pelvis)
c)nuclear medicine scans, (bone, gallium, brain, etc.)
d)any GI tract studies, (upper GI series, barium enema, small bowel follow through)
e)lymphogram
