Below is a printable PET/CT patient referral form and patient information sheet for scans to be done only at the BC Cancer Agency. To refer a patient for a PET/CT scan, please print the referral form, fill it out completely, and fax it back to our office.

If the request meets criteria for the BC Cancer Agency's current clinical indications, our receptionist will contact the patient as soon as possible to book a scan date and time as well as go over any prescan instructions.
