The HPV FOCAL team recognizes the contribution of Medical Office Assistants (MOAs) in the Family Practice Clinics, to the study.

Please call us, if you are unsure when a patient is due for her next study cervical screen exam.
















The HPV FOCAL Study Gynecological Requisition form is required for study samples. As per BC Cervical Cancer Screening Program guidelines, all cervical samples are to be labeled with the patient's last name AND date of birth, or they will be rejected at the laboratory.

If extra supplies are required, the Study Supply order form should be filled out and faxed to the study centre.
