A patient’s date of birth (DOB dd/mm/yyyy) + their last name is now required on Pap sample slides. Please note, Pap samples will be rejected if both identifiers are not present on the slide after FEB 1, 2012. |

First time submitting your samples?
Practitioners and clinics submitting gynecological cytology samples for the first time or needing to update their contact information should contact the lab before sending in samples. Phone 1-877-747-2522 or fax to 604-708-8027.
