Breast Cancer Nurse Navigation Program
This is a pilot program for patients with biopsy proven breast cancer. Breast Cancer Nurse Coordinators will be available to provide support to patients and care givers in addition to the existing cancer care structure. The Breast Cancer Nurse Coordinator is focused on patient centred care. Among others, the coordinator can assist in coordinating care, assist patients to understand breast surgery and treatment options, and provide information and support to patients throughout their decision-making process. This partnership with the patient begins with the diagnosis of breast cancer, continues through any treatments and into the follow-up phase of care. The form of contact is determined by the patient. It could be phone calls, face to face, e-mail, etc.
Referrals can be made by the family physicians, surgeons, other health care providers, the patient themselves or family members. Please use this form: FormBreastCancerNurseNavigationClinicreferralnew05April2011FINAL.doc
