The BC Cancer Agency is dedicated to providing you with optimum care. For this reason, you and your family doctor will be asked to supply personal information related to your treatment program.
By law, the BC Cancer Agency must comply with the requirements of British Columbia's Freedom of Information & Protection of Privacy Act.
Your personal information will only be disclosed for the following purposes:
- your care
- maintaining contact with you to help us improve our quality of care
- teaching, education and quality control
- as directed under the law
- as authorized by the Act
When necessary, relevant reports will be sent to your referring physician or other health care agencies/facilities involved in your care to ensure continuous and consistent medical treatment.
Verbal or written information will not be disclosed to any other parties without your consent. An authorization for Release of Information form can be completed by yourself at the BC Cancer Agency, Centre for the Southern Interior to authorize us to provide information to those parties you have indicated, ie. family member(s), friend(s).
