  3. Laboratory reports
    1. CBC
    2. creatinine, uric acid, BUN, calcium, bilirubin, SGOT, alkaline phosphatase, lactic dehydragenase (LDH), protein electrophoresis
  4. Information re: other malignancies
  5. Imaging reports
    1. chest X-ray
    2. CT scan, (brain, thorax, abdomen, pelvis)
    3. nuclear medicine scans, (bone, gallium, brain, etc.)
    4. any GI tract studies, (upper GI series, barium enema, small bowel follow through)
    5. lymphogram
