Lung Screening Eligibility Assessment Request Form (Provider Use)

Used by primary care providers to request the Lung Screening Program contact a patient for an eligibility assessment. ‎

Lung Screening Referral Update Form (CT Department Use)

If the CT department is unable to proceed with the CT scan due to being unable to contact the patient, or if the patient is being transferred to a different site for their scan, the CT scan department should complete the Referral Update Form to notify the Lung Screening Program.
