Please use the form corresponding to the desired scan location. PET/CT scanning is available to those who meet specific approved indications and guidelines.

To refer a patient for a PET/CT scan in Vancouver, Kelowna , or Victoria, please print the location-specific re​ferral form, fill it out completely, and fax it back to our corresponding office at:

  • Vancouver: 604-877-6245
  • Kelowna: 250-861-6459
  • Victoria: 250-519-5458
To refer a patient for UGIPRRT, please fill out the referral form completely and fax it back to the corresponding office:
  • Patients outside BC Cancer System: 604-708-2005
  • Patients at non-Vancouver BC Cancer Sites: 604-642-8888
