Access your health records online or request records from BC Cancer.

If you are requesting your own health records, you can submit your request using the online form.

If you are requesting someone else’s health records, you must complete and submit the Authorization for the Release of Health Records form. You can also use this form to request your own records if you prefer not to use the online form.

Submit the form

​Email, fax, mail or drop off the completed form to the Health Information Management Release of Information Office at the BC Cancer centre or hospital that holds the records you are requesting.

You can also request access to your health records by contacting the Release of Information Office at the regional cancer centre where you receive treatment.

Access health records online

You may already be able to view some of your health records online through provincial and regional health services.

Health Gateway

Health Gateway is British Columbia's patient portal. Through Health Gateway, you can access:

  • Lab results
  • Medical imaging reports
  • Medications
  • Immunization history
  • Health visits
  • Hospital visits
  • Special Authority requests
  • COVID-19 proof of vaccination
  • Organ donor registry information
  • BC Cancer screening letters

To access your health records, ​visit the Health Gateway​ portal.

AccessMyHealth

AccessMyHealth is an online service that provides access to some electronic health records from most BC Cancer locations. (Kelowna records are not yet available.) 

If you are a BC resident aged 12 or older with a BC Services Card account, you can view:

  • ​Upcoming and past appointments
  • Lab, medical imaging, and other diagnostic results
  • Allergies
  • Procedure history​​
  • Messages from your care team
  • Select clinical documents, including discharge summaries, clinic notes and patient handouts

Proxy access

AccessMyHealth also offers prox​y access​, which allows approved family members or caregivers to view a patient's health records.

To set up proxy access, you must complete an authorization process.
