    1. original diagnostic biopsy

    3. any fine needle aspirate biopsies done to confirm diagnosis
  2. Operative reports
    1. operative description of the original diagnostic procedure
    2. any fine needle aspirate biopsies done to confirm diagnosis
  3. Laboratory reports
    1. CBC
    2. creatinine, uric acid, BUN, calcium, bilirubin, SGOT, alkaline phosphatase, lactic dehydrogenase (LDH), protein electrophoresis
  4. Information re: other malignancies
  5. Imaging reports
    1. chest X-ray
    2. CT scan, (brain, thorax, abdomen, pelvis)
    3. nuclear medicine scans, (bone, gallium, brain, etc.)
    4. any GI tract studies, (upper GI series, barium enema, small bowel follow through)
    5. lymphogram
