Survivorship & Primary Care

Survivorship is a key component of the cancer care continuum. It is the link between treatment and recovery and between the patient, the cancer agency, and their primary care team.
Survivorship resources and services are intended to help a person cope with their new normal, while also ensuring their health care providers have the education, tools, and resources they require to provide high quality care to cancer survivors.
